Notice of Privacy Practices
How Elev8 Drip Hydration Therapy may use and disclose your protected health information — and your rights regarding it.

Elev8 Drip Hydration Therapy (“Elev8,” “we,” “us,” or “our”) is committed to protecting the privacy of your health information. This Notice applies to the protected health information (“PHI”) we create or receive in connection with your care.
1. Our obligations
We are required by law to:
- Maintain the privacy and security of your protected health information;
- Provide you with this Notice of our legal duties and privacy practices with respect to your PHI;
- Notify you following a breach of unsecured PHI; and
- Follow the terms of the Notice currently in effect.
2. How we may use and disclose your PHI
We may use and disclose your PHI, without your separate authorization, for the following purposes:
Treatment
To provide, coordinate, and manage your care — for example, our clinicians reviewing your intake and screening information to select and safely administer an appropriate treatment.
Payment
To obtain payment for the Services you receive — for example, processing your payment method or documenting the services rendered.
Health care operations
To support our business operations — for example, quality assessment, staff training, and clinician review of the care provided.
Appointment reminders & follow-up
To contact you with appointment reminders, confirmations, and post-visit follow-up regarding your care.
As required or permitted by law
We may use or disclose PHI when required by federal, state, or local law, and for purposes such as public health activities, reporting suspected abuse or neglect, health-oversight activities, judicial and law-enforcement requests as permitted by law, to avert a serious threat to health or safety, and for workers’ compensation as authorized.
3. Uses that require your authorization
Other uses and disclosures — including most uses of psychotherapy notes (if any), uses for marketing, and any sale of PHI — will be made only with your written authorization. You may revoke an authorization in writing at any time, and we will stop using or disclosing your PHI for those purposes, except to the extent we have already relied on it.
4. Your rights regarding your PHI
You have the right to:
- Inspect and copy your PHI held in our records, subject to limited exceptions;
- Request an amendment of PHI you believe is incorrect or incomplete;
- Request an accounting of certain disclosures we have made;
- Request restrictions on how we use or disclose your PHI (we will accommodate a request to restrict disclosure to a health plan for items you paid for in full out of pocket);
- Request confidential communications by alternative means or at an alternative location;
- Receive a paper or electronic copy of this Notice on request; and
- Be notified if a breach of your unsecured PHI occurs.

5. Changes to this Notice
We reserve the right to change this Notice and to make the revised Notice effective for PHI we already have as well as information we receive in the future. The current Notice will be posted on our website with its effective date, and a copy is available on request.
6. Complaints
If you believe your privacy rights have been violated, you may file a complaint with us using the contact information below, or with the U.S. Department of Health and Human Services, Office for Civil Rights. We will not retaliate against you for filing a complaint.
